Your Initial Visit
Our goal is to provide you with the very best care. At your initial appointment, your dental clinician will discuss your questions and concerns and complete a comprehensive examination including an oral cancer examination. The appropriate diagnostic records will be recommended. These may include low radiation digital radiographs, diagnostic models of your teeth, and photographs. After the doctor has reviewed your records and thoroughly evaluated your individual findings and concerns he will formulate a comprehensive plan. At a subsequent consultation your treatment options and the pro’s and con’s of each option will be discussed in detail.
Please assist us by providing the following information at your initial appointment:
Your Health History Questionnaire and a list of current medications.
Any recent radiographs or any other information you feel may be helpful.
IMPORTANT: A parent or guardian must accompany all patients under the age of 18.
Please alert our office if you have a medical condition that may be of concern (e.g. diabetes, high blood pressure, artificial heart valves and joints, rheumatic fever, etc.), if you are taking any medications, (i.e. heart medications, aspirin, anticoagulant therapy, etc.), or if you require antibiotics prior to dental cleanings.
If your previous dentist has taken recent x-rays (within 6-months), please request that these be forwarded to our office.
